Print and complete the application form and photo release form below.
Step 3
The enrollment process must be completed by the Friday before class starts. This includes forms, payments, identification and the initial TB step.
Bring your CDC COVID Vaccination Record (required by clinical site), your completed application form, your signed photo release form, Social Security card and Government issued photo identification at the time of your appointment.

This booklet is REQUIRED for all students and contains important information on state requirements and how to schedule your STNA test.
